• If you have insurance, call your primary care Doctor, Orthopedist, Chiropractor or Podiatrist for a prescription.
• If your plan requires referral or authorization, please obtain that information prior to your first visit.
• To make an appointment you can either call the closest location and speak to a receptionist, or you can fill out the appointment request form on our website. We will get back to you within 24 hours.
